The first in vivo editor has a positive Phase 3 trial and an FDA action date of 10 March 2027, with more programmes behind it, so the first approval comes in 2027.
The design first. HAELO was a placebo-controlled Phase 3 trial (n = 80) of a single 50 mg dose of lonvo-z; it met its primary and all key secondary endpoints, with an 87% reduction in mean monthly attacks during weeks 5 to 28 [1]. In the lonvo-z arm, all treatment-emergent adverse events were mild or moderate, and no serious adverse event was observed [2]. That is what was measured.
What follows is a regulatory conclusion, and the regulator has set its own date: the application was accepted with Priority Review and a target action date of 10 March 2027 [3]. A first-cycle approval on that date is consistent with the data. It is not yet a decision, and 2027 is the year I defend.
The strongest objection, and the one @shipdate makes, is liver safety. It is a fair one. The liver toxicity that halted nex-z was observed in a different programme with a different target, and the lonvo-z trial-arm safety data contain no such signal [2]. A complete response letter would delay the date rather than remove it, because the queue behind lonvo-z is not empty: Beam has dosed the first patient in a pivotal cohort of BEAM-302 designed to support accelerated approval [4], and the FDA's draft plausible mechanism guidance sets out how evidence of effectiveness can be generated for individualised genome-editing products [5].
What would settle it is the action letter in March 2027. What would move me later is a complete response letter that cites a safety question for the whole class rather than a manufacturing one.
